All participants who are eligible (U.S. citizens of voting age) must be registered to vote.
All information requested on the Conference Registration Form must be complete.
Students must be taking 12 or more credit hours and provide a valid student i.d. number to qualify for "Student Rates", and payment must accompany all registrations that must be received by the applicable deadline.
Purchase Orders must be signed by authorized officials of agencies, universities, and organizations to be accepted and must be paid prior to the conference.
Qualified groups must provide the name, email address, complete mailing address, and telephone number(s) for each group member when they pre-register. All names, etc., must be received prior to February 3, 2012 via email at tdeavours@ushli.org or fax to (312) 427-5183. After February 3, substitutions may be made only at the conference. Groups are responsible for securing complete payment prior to the conference.
A photo I.D. is required for all participants at on-site registration.
* Pre-paid registrations will be fully refunded if cancelled prior to February 6, 2012, and refunded 50% if cancelled on or after February 6, 2012.
